Sandy Koufax (like Walter Johnson) refused to ever intentionally throw at a batter, under any circumstances.

Dodgers catcher John Roseboro, angered by Giants pitcher Juan Marichal brushing back two of his teammates earlier in the game, and frustrated by his pitcher's refusal to throw at Marichal, threw several balls back to Koufax that nearly hit Marichal's head.

Marichal took exception to Roseboro's throws, and all hell broke loose.
