Fresh to the dugout doesn't really mean fresh.

 

It's not about the total number of innings, it's about managing who has pitched a lot of innings recently. Taxi squads and optioning guys back and forth are common parts of the game. I agree that fresh arms aren't necessarily good arms but you still need to have guys available for blowouts and extra innings.
