I think I figured it out.  MLB.tv delays the radio feed so they can play more ads.  My radio feed always ends up behind and sometimes they cut into the start of innings with more ads.

Yeah, they used to have minimal delay back in the early 2010s (like maybe five seconds) but after they updated to HD, the streaming for TV and radio both picked up 30ish second delays.  Been that way ever since, as far as I know.

The scoreboard initially showed a hit. It was changed.
